I noticed that Watto, the guy who made the mod, hasn't posted here in 2 years, so not much chance getting help via a private message.
If I knew where the levers were that unlock the door, I could edit the file with a hex editor. Does anybody know?
There should be a patch for a major bug like this.

First off, the comment from Brian about "Microsoft approval" does not apply to your problem. That was from a thread about XBLA issues. Clearly you are using the PC version. Microsoft does not have squat to say about any PC patches, but your post indicates that you have already installed the latest and last TL1 patch, 1.15.
1) if you have any nods at all installed, uninstall them, return to the floor and see if all is working.
2) The levers go, just like any other single instance boss related effects, when the boss has been triggered / defeated. The door should remain open, leading into the antechamber witha broad set of steps leading down to a waygate and the first stairs to the Black Palace levels. From your description, this isn't what happened for you, possibly because pet or other effect killed Medea just as you were retreating upstairs.
a) If Medea is defeated, level cleared, levers gone - and the door is still closed, try just approaching the door to see if it will move.
b) If that doesn't work, go back to town, go to the waygate, and see if you have a gate option for the Lost Fortress level already. If so, use it. The antechamber with the waygate and the stairs is a non-monster area as far as I know.
c) If that doesn't work, since you don't want to install any mods that you consider risky, you could go back up to 28, establish your town portal there, go back to town. Re-run levels 1-3 quickly - it seems to help reset the respawn process - then portal back to 28 and walk down to 29. There's a good chance it will have respawned. Note that as long as you keep a town teleportal open on floor 29, it WILL NOT RESET.
(I am not entirely sure that floor 29 will ever respawn, and if it does, you may not get Medea, since it is a special boss level, but it is worth a try. If it does reset, be sure to pull the lever nearest the door first, then set yourself and pull the one further away once you are ready, to avoid mobbing.)

I would not worry about using Watto's level reset lever mod. The PC game engine has not been changed since the mod was posted. Be sure to use the URL at the end of the thread, not the link that says "here".
(Caveat: I only know this error and the mod fix through reading / reputation; I have never had the problem.)

With respect to how common the Lost Fortress problem is, I think your knowledge of such things is far beyond mine, and as such, your judgments should be heeded.

BTW, when you get there, don't heirloom anything with a +HP enchant. Bad stuff happens. Search on "heirloom bug for details.
